FMCSA regulation 49 CFR §391 requires motor carriers to maintain a DQ file for every driver. It must include the application for employment, MVR, road test certificate, medical card, annual review and Clearinghouse query, among other items. Missing or incomplete DQ files are one of the top three findings in DOT audits.
